During your first visit with us, you’ll arrive 10-15 minutes early to fill out your new patient paperwork. Getting to know you and your medical and dental history is important to us, as it helps us provide the best treatment options possible for you. You’ll also want to bring a photo ID and insurance card (if paying with insurance).

We do not accept Medicaid or any HMO plans at this time.
